This is written by a senior member of the operations team after consultation with all parties. The author is also always a registered nurse or paramedic.

It outlines event risk demographics such as attendee numbers and specific event risks, with formal risk scoring using nationally recognised systems such as the Event Industry Forum (Purple Guide), Green Guide and relevant athletic associations. It defines staffing levels including numbers and skill mix, along with resources such as medical areas, ambulances, response cars and cycle response units. The plan also details major incident procedures, how initial management is handled and how liaison with the NHS is coordinated, how operational decisions are made jointly with NHS services on site, and provides key contact numbers and radio call signs for effective communication.
